Online Check-In Forms

Van Stavern Small Animal Hospital offers optional patient check-in forms online so you can complete them in the convenience of your own home or office. You do not need to print the form. Our check-in forms increase the efficiency of your drop-off process and reduce communication errors. Please schedule your appointment prior to filling out your information, so that we may direct you to the appropriate form. Allow for 5-10 minutes at the time of your drop-off for staff to confirm your check-in.

If you do not wish to check in online, please allow for 15-20 minutes at the time of your drop-off for staff to go over patient history with you.
